Dementia Care Services

|
Living with the experience of dementia can be challenging. Whether you
have a diagnosis of dementia yourself, or are caring for a person who has
a suspected or diagnosed dementia, we may be able to be of help to you. At Age Concern Preston and South Ribble we are developing a range of pioneering dementia services; this is in response to the individual needs and wishes of people whose lives are affected by memory concerns and the people who support them. The following services are now provided: 1. Flexible Outreach Service enabling people with memory concerns to maintain independence and achieve wellbeing through meaningful activity and the pursuit of interests. 2. Withy Trees Day Support Centre providing practical emotional and personal care for older people living with dementia and offers respite for their carers. 3. Citizen Advocacy Scheme empowering people with dementia who live in care homes; to have their voices heard and their rights, needs and wishes represented.
