Flexible Outreach Service enabling people with memory concerns to maintain
independence and achieve wellbeing, through meaningful activity and the
pursuit of interests Who can receive this service? • Any person aged 65 or over with a suspected or diagnosed memory problem, living in the Preston or South Ribble area, who could gain benefit from activity focused support. • People with early onset memory concerns under 65 years of age may also benefit from this service. The service aims to: • Enable individuals to continue to enjoy interests and hobbies • Help people to live as independently as possible in their own home • Provide emotional support and companionship • Enhance social opportunities for individuals in their local community • Promote self esteem, confidence and enhance well being • Provide information and guidance to service users and their carers Who is involved in providing the service? Age Concern Preston and South Ribble has trained and experienced Flexible Outreach Support Workers and Volunteers involved in working with people on an individual basis. For how long is the service provided? This service is planned to meet a persons specific needs and will be reviewed on a regular basis.
