We deliver a number of services which promote independent living for people 55 or over, their carers, family and friends.
This range of services is provided to meet the needs of people with lower level needs and some services are available to those who are physically active and living independently. Most services are free at the point of access, however some require a contribution from the user for the costs e.g. a charged-for Home Help service, Access to Benefits.
The core purpose of these service are to focus on rehabilitation, re-enablement and avoidance of unnecessary hospital admissions, enablement for the individual, the reduction of social isolation, the maintenance of skills, making informed choices and having the confidence to take control, whilst maintaining independent living and promoting quality of life whenever possible.

Promoting Independence: Preventative support for older people to help them to remain independent across Preston, South Ribble, Chorley and West Lancashire.
The service provides both low level practical and emotional support at a time of crisis or to avoid a potential crisis, additional ‘hands on’ support to help maintain or develop daily living skills and to then further promote links back into the community to enable social interaction and promote engagement in activities and local groups.
This service could help if you or anyone you know is struggling with daily living tasks and may need support, for example, due to: a bereavement, fall or be at risk of an injury, hospital discharge, recent illness and is struggling to become independent again, loss of confidence, and/or loneliness or social isolation.
As part of the Promoting Independence service we can offer the Take Home and Settle service: this offers practical help and support, delivered with respect, dignity and compassion for those normally independent and living at home. We work with the service user to help them regain independence following discharge from hospital and reduce the risk of re-admission.
Take Home and Settle is aimed at those resident in Chorley, Preston, South Ribble and West Lancashire, who are being discharged from the Lancashire Teaching Hospitals NHS Foundation Trusts sites at Royal Preston and Chorley and South Ribble Hospital.
What support can we offer?
Immediate emotional and practical support such as:
- Assistance with hospital discharge including transport home when patient transport not available.
- Initial food shop on discharge from hospital.
- Ensuring a safe home environment.
- Making a drink and preparing a snack.
Also further practical support such as:
- Assistance to get out and about to complete essential tasks such as shopping.
- Assistance to undertake daily living skills such as bedding change.
- Referral to other sources of help as appropriate, such as benefit checks, befriending services and checks to ensure the home is able to maintain a comfortable temperature.
- Telephone support.
