Martha is a delightful, chatty 84 year old lady who lives alone and copes relatively well with the assistance of carers. Recently she has become more confused and frail causing concern to her family over her wellbeing. Martha attended a weekly church event but other than this link, she has no regular social involvement. Martha’s family were concerned that she was in danger of becoming socially isolated and made a referral to Dementia Community Links, to help promote social contact away from her home environment, possibly through day support. After spending time in getting to know Martha I discussed the benefit of day support, and arranged an accompanied visit to the Leyland Centre’ as this was local to where she lived.
