Lancashire Wellbeing Service Vacancies, Age Concern Central Lancashire is looking to recruit, three Wellbeing Workers
1 x 36 hours (Chorley and South Ribble)
1 x 36 hours (West Lancashire)
1 x 36 hours (Lancaster and Morecambe)
Salary £20,456.00 per annum
This service supports vulnerable adults, living with long term health conditions and at risk of a health or social care crisis, to stay well and maintain their independence.
Three voluntary organisations: n|compass, Age Concern Central Lancashire and Richmond Fellowship have come together in a consortium to provide an integrated service delivery model for Lancashire County Council.
The consortium was forged through a collective desire to offer something truly unique to the people of Lancashire. We believe that, through harnessing the expertise of each individual organisation, we have created an innovative model far stronger than the sum of its parts; one that delivers effective, high quality services to the people and communities we aim to serve in Lancashire.
This service targets and supports adults (18 years plus) to aid or improve their mental, physical and social well-being and, by doing so, enables people to build resiliency and to lead healthy, informed and independent lives. The service works closely with a range of organisations to access and support adults across Lancashire.
