Earlier this week the Lancashire Evening Post reported that Lancashire is the worst in the North West for a rise in 'bed blocking' in hospitals (click here for the story). We though we would take the opportunity to tell you how we can help alleviate 'bed blocking' with a service provided by our Promoting Independence Team.
We hold the Crisis Aversion contract, which is delivered across the Central Lancashire area. It is funded by Lancashire County Council to deliver low level preventative support to older people to help them to remain independent. The remit of the service includes working with local hospitals and to support hospital discharge by assisting older people when they return home with practical and emotional support to promote independence and prevent readmission to hospital. The service provides immediate practical support for example shopping, urgent errands such as collecting prescriptions, help to arrange any follow up medical appointments and to make sure the home environment is safe and warm in winter.
The service is available Monday – Friday 9.00 to 4.30 (excluding bank holidays).
